- Dressing and undressing
- Toileting and incontinence care
- Changing clothing and linens
Mobility & Positioning
CHHAs may assist patients with:
- Safe transfers
- Getting in and out of bed
- Positioning and repositioning
- Ambulation, as directed
- Use of prescribed assistive devices, as trained
- Maintaining a safe environment
Comfort & Daily Living Support
Our CHHAs may assist with:
- Maintaining a clean and safe patient area
- Changing bed linens
- Basic personal care
Family & Caregiver Support
CHHAs work with patients and caregivers by:
- Following the established plan of care
- Communicating patient needs and changes to the appropriate Home Health RNCM and team member
- Reinforcing instructions provided by the nurse or therapist, within the aide’s scope of practice
- Supporting safe personal care routines
Promoting Dignity & Independence
Our CHHAs provide care with respect for each patient’s privacy, preferences, culture, and dignity. We encourage patients to participate in their care as much as safely possible and support independence in daily activities.
Part of Your Home Health Team
CHHA services are provided under the direction and supervision required by the patient’s home health plan of care. Our aides work as part of the Home Health Team alongside nurses, therapists, physicians, social workers, patients, and caregivers.
Individualized Care: CHHA services are provided based on the patient’s individualized plan of care, applicable physician or allowed-practitioner orders, medical necessity, and applicable Medicare/CMS and state requirements. Services are limited to tasks within the CHHA’s permitted scope of practice.
