Caregiver Support & Education

MSWs may assist families and caregivers with:

Understanding available resources
Caregiver support and coping strategies
Identifying additional support needs
Planning for changes in the patient’s care needs
Connecting caregivers with appropriate community services

Care Planning & Coordination

Our Medical Social Workers collaborate with the healthcare team to:

Identify psychosocial barriers to care
Support the patient’s individualized goals
Connect patients with appropriate resources
Coordinate with community agencies when appropriate
Participate in care planning
Communicate relevant concerns to the healthcare team
Support safe transition and continued care at home

Support During Life Changes

MSW services may be helpful for patients and families experiencing:

A new diagnosis or change in health

Helping Patients Stay Safe at Home

Our Medical Social Workers focus on helping patients and families identify practical solutions, access appropriate resources, and address psychosocial barriers that may affect the patient’s ability to remain safely at home.

Individualized Care: Medical Social Work services are provided based on medical necessity, physician or allowed-practitioner orders, the patient’s individualized plan of care, and the specific needs of each patient and family.

Our MSWs work as part of the Home Health Team alongside patients, families, caregivers, physicians, nurses, therapists, and other healthcare professionals to support coordinated, patient-centered care at home.

The representative who assisted us was exceptional and very knowledgeable. You could tell that they really care and treat you like family. Lots of promises, no delivery, no care in the world that your loved one isn’t getting the services that Medicare is paying Pacific Point to provide. They don’t have enough employees to deliver. The care I received was above and beyond anything I thought possible.  The in home health Care from Edith, the clergy, Edwin, Jenny the Social Worker, and Shawn, my case manager.   Shawn was my nurse as well, he was with me every step of the way and was more than I could have hoped for.  His professionalism, compassion, experience, and strength kept me calm. His ability and concern was touching, surprising and comforting to me and my family.  His calming control made  the journey for my mom and myself a beautiful experience.   I can honestly say I love him for what he did.  When my mom passed he was there helping, making calls and taking the pressure away from me.  He is a gift, really.  He honors the company he works for.
